Complete the Data Inquiry Form

- Enter the Date
- Name
- Agency information
- Contact information
- Check "Childhood Lead Poisoning" for Data Requested
- Select "Submit"

Log In to the Data Request System
|
- Enter Log-in Information
- Select "Log In"
|
User Dashboard and Active Projects
The Default view on the User Dashboard Shows All Active Projects
|
- Select "Manage" under the existing Childhood Lead Submission Project
|
Project Details: Folders
Within the Childhood Lead Project there are Established Folders
|
- Select "Manage" within the folder labelled as "Childhood Lead Data Submissions"
|
Project Details: Files and Messages
Within the Childhood Lead Data Submissions Folder there are Files and Messages
|
 |
- Select "Add New File" to Upload Data
- To upload multiple files at once you will need to have all the files you intend to upload within a zip folder that you create and then upload that zip folder to the DRS

- Select "Add Message" to add any special instructions for the Childhood Lead Poisoning Data Steward

- The system administrator will confirm receipt of the data submission via email or messages throught the application.
